2. Carefully inspect the work area and identify potential hazards.

3. Document all hazards, regardless of how minor they may seem.

Remember: If you encounter a hazard that cannot be adequately controlled, STOP WORK and notify your supervisor immediately.

Ladder Inspection Form

This form is to be used by all Bulletproof Roofing employees to conduct a thorough inspection of any ladder before each use. Ladders are essential tools in our trade, but they can also be hazardous if not properly maintained and inspected.

Instructions:

1. Inspect the ladder before each use.

2. Carefully examine all parts of the ladder, including the rungs, rails, feet, and locking mechanisms.

3. Document any defects or damage, no matter how small.

4. If any defects are found, remove the ladder from service immediately and notify your supervisor.

5. Sign and date the form after completing the inspection.

Remember: Never use a damaged or defective ladder. Your safety is our priority!

Fall Arrest Plan

This form is to be used by all Bulletproof Roofing employees to develop and document a comprehensive fall arrest plan before beginning any work at heights. Working at heights presents significant risks, and a well-defined fall arrest plan is crucial to preventing falls and ensuring worker safety.

Instructions:

1. Complete this form before starting any work at heights.

2. Identify all potential fall hazards in the work area.

3. Select and document the appropriate fall protection systems to be used. This may include guardrails, safety nets, personal fall arrest systems (PFAS), or a combination of methods.

4. Specify the anchor points for PFAS, ensuring they are strong enough to withstand the forces of a fall.

5. Outline rescue procedures in the event of a fall.

6. Communicate the fall arrest plan to all workers involved in the task.

7. Ensure all workers are trained in the proper use of fall protection equipment.

8. Review and update this plan as conditions change throughout the workday.
